Student Registration and Annual Update Option 1

This option is for parents or guardians who DO NOT currently have a student enrolled in a Banks County School and wish to enroll a new student.  If this does not describe your situation, please return to the Student Registration and Annual Update landing page by clicking HERE and select the option that best descibes your situation.

Online Registration Procedures

  1. Access the online registration portal for New Students at the bottom of this section.
  2. Enter any the requested information including your email address.  AN EMAIL ADDRESS IS REQUIRED.
  3. You will receive an email with a link to your registration form for you to complete.  DO NOT DELETE OR SHARE THIS EMAIL.  This email contains a unique link for your registration form.  If you need to pause and complete your form later for any reason, you will not be able to access your form without this link.
  4. After you have completed and submittied you student's registration form, schedule an appointment with your child's school to complete the enrollment.
  5. During your appointment, make sure to bring the required documents listed below. We must have these to complete the registration process.  If you have uploaded electronic copies, we will need these to verify the uploads.

​Required Documents

  • Proof of Residence  
    • For Homeowners any of the following will be accepted along with a Photo ID provided the document(s) include the name of the enrolling parent/guardian:
      • A current residential property tax statement
      • Purchasing/closing documents for the residence
      • A current electrical, landline telephone (no cellular), or other utility bill
    • For Renters any of the following will be accepted along with a Photo ID provided the document(s) include the name of the enrolling parent/guardian:
      • A current lease/rental agreement consisting of written evidence that the agreement is valid
      • A current electrical, landline telephone (no cellular), or other utility bill
    • For Non-Renters or Occupants of another person’s home residence any of the following will be accepted along with a Photo ID provided the document(s) include the name of the enrolling parent/guardian:
      • A designated Banks County Schools Property Affidavit signed by the parent/guardian and property owner. Please click HERE to download.
        • NOTE: The property owner must also provide documentation required for homeowners enrolling students
  • Copy of Birth Certificate (for each registering student)  
    • Birth Certificates with the biological parents noted may be requested from the Georgia Department of Human Resources Vital Records at 404-679-4701.  Birth Certificates for children born in other states may be obtained by contacting the Vital Records Department within the state of birth.
      • NOTE: Hospital Certificates are NOT acceptable.
  • Immunization Certificate (for each registering student)  
    • A current and completed Georgia Department of Human Resources Form 3231 is required by state law showing immunization for Measles, Mumps, Rubella, Polio, Whooping Cough, Tetanus, Diphtheria, Hepatitis B, and Varicella.  This form may be obtained by taking the current immunization record to the Banks County Health Department (706-677-2296) or a Georgia physician.
  • Certificate of Ear, Eye, and Dental Examination (for each registering student)  
    • A current (within one year of initial school entry) and complete (ear, eye, and dental exam) Georgia Department of Human Resources Form 3300 is required by state law.  This form is available from the Banks County Health Department (706-677-2296) or from a private physician.
  • Original Social Security Card  
  • Student Records  
    • Withdrawal Form, Transcript(s), and Special Education Records
      • Required if enrolling after the beginning of the school year.  It must include grades, courses, standardized test scores, and any information on any special education services.
    • Discipline and Criminal Record Inquiry  
      • This form is required by state law and must be completed and signed for all students enrolling in grades 6 – 12.
    • Attendance History
    • In lieu of records, a records request affidavit may be submitted so that Banks County Schools may conditionally enroll the child and request records from the child’s previous school.  You will have the opportunity to agree to this during the registration process.
  • Guardianship/Custody Information and Photo ID
    • If the enrolling student is living with someone other than a birth parent or legal guardian, documentation is required to support this arrangement
    • In cases of divorce, a copy of the divorce decree is needed. ​
